Baby Teething and Sleep: What to Expect and How to Help

Your baby was on a roll. Longer stretches, predictable naps, a bedtime routine that actually worked. Then the drooling started, followed by the fussing, followed by nights that look nothing like they did last week.
Teething gets blamed for almost everything in the first year — and sometimes the blame is warranted. But "teething" is also one of the most over-applied explanations for baby sleep disruption, which means parents sometimes ride out weeks of genuinely fixable sleep problems while waiting for teeth to pass.
Here's what's actually happening, when it happens, and what you can do that will genuinely help.
When Teething Starts and What the Timeline Looks Like
Most babies get their first tooth somewhere between 4 and 7 months, though the normal range extends from 3 months to 12 months. The full set of 20 primary teeth arrives in waves over the first 2.5–3 years.

Sleep disruption isn't a constant during all this time. Most of the teething timeline is quiet. The periods that cause the most disruption tend to be when teeth are actively cutting through the gum — typically 3–7 days before the tooth emerges. Between eruptions, sleep usually returns to baseline.
The second molars at around 2 years are often the most disruptive because of their size and the intensity of pressure involved as they break through.
Why Teething Disrupts Sleep
Most parents associate teething with drooling and biting — but the main mechanism behind sleep disruption isn't the drool. It's inflammation.
When a tooth is pushing through the gum, the tissue responds with localized inflammation — swelling, increased blood flow, and a release of inflammatory chemicals. This is the same process that makes a splinter hurt far more than its size would suggest. The inflammation activates pain pathways that are genuinely uncomfortable, and discomfort wakes babies up just like it wakes adults up.
What makes teething specifically disruptive to sleep (versus just daytime fussiness):
Lying flat increases pressure. When your baby is upright, gravity helps pull blood away from the inflamed gum tissue. When they lie flat, there's more blood pooling to that area — which increases the sensation of pressure and pain. Many teething babies who seem fine during the day will wake at night because the horizontal position intensifies what they're feeling.
Sleep cycles are the vulnerability point. Babies naturally surface from sleep at the end of each sleep cycle (roughly every 45–60 minutes). Adults do this too, but we typically fall right back asleep without fully waking. A baby with gum discomfort is much more likely to fully wake at that surface point rather than drift back into the next cycle.
It's not manipulation. Teething waking is genuine biological discomfort. Your baby isn't waking because they want attention or have learned a habit — though habits can develop secondarily if teething nights lead to new sleep associations. It's worth keeping that distinction clear so your response matches the cause.
Is It Teething? How to Tell
Because teething is blamed for everything, it helps to know what it actually looks like versus what it doesn't.
Signs consistent with teething:
- Swollen, red, or visibly raised gum tissue (you may be able to see the tooth bulging beneath)
- Increased drooling (though drooling alone doesn't mean teething — babies drool a lot generally)
- Gum-biting, finger-chewing, or the need to put everything in the mouth
- Fussiness that starts 2–5 days before a tooth appears, then improves once the tooth breaks through
- Low-grade temperature elevation (true fever above 100.4°F is NOT a teething symptom — more on this below)
- Disrupted sleep that coincides with visible gum changes
Signs that suggest something else is going on:

The most important rule: teething doesn't cause fever. The persistent belief that it does has led to parents missing genuine illnesses. If your baby has a temperature above 100.4°F, treat it as a fever — not teething — and consult your pediatrician if you're unsure.
What Actually Helps
Before Bed: Cold Teethers
The cold is doing two things: reducing inflammation (the same reason you put ice on a sprain) and providing counter-pressure on the gum, which temporarily blocks the pain signal. A chilled (not frozen) silicone or rubber teether given 15–20 minutes before your sleep routine can take the edge off enough to help your baby settle.
Important: Freeze teethers are not recommended — frozen teethers are too hard and too cold, and can damage delicate gum tissue. Refrigerate, don't freeze.
Wet, chilled washcloths work well for younger babies who aren't ready for traditional teethers. Let your baby chew and gum it for a few minutes before starting the wind-down.
Pain Relief: Acetaminophen or Ibuprofen (Age-Appropriate)
For genuine teething pain that's disrupting sleep, age-appropriate pain relief is appropriate and safe when used as directed.
- Acetaminophen (Tylenol): Safe from 2 months and up, dosed by weight. Effective for teething pain.
- Ibuprofen (Motrin/Advil): Safe from 6 months and up, dosed by weight. Anti-inflammatory in addition to pain-relieving, which may make it more effective for teething specifically since inflammation is the root cause.
Giving a dose 30–45 minutes before your bedtime routine — when you know a particularly rough teething period is active — can meaningfully improve night sleep. Use the dosing chart on the packaging, dosed by your baby's current weight.
Topical gels and numbing agents: These have fallen out of favor, and with good reason. Benzocaine products (like Orajel) carry an FDA warning for children under 2 — benzocaine can cause methemoglobinemia, a rare but serious blood oxygen problem. Natural teething gels (chamomile-based, homeopathic tablets) have not been proven effective and some (belladonna-containing products) have been recalled for safety concerns. Stick with cold and appropriate pain medication — they work better anyway.
The Bedtime Routine Doesn't Change
One of the most important things you can do during a teething stretch is maintain your existing sleep routine. A consistent, predictable routine provides your baby's nervous system with the signal that sleep is coming — even when discomfort is present. Abandoning the routine because "they're teething anyway" disrupts the behavioral cues your baby has learned to associate with settling.
If you're dealing with a bad teething night, it's fine to offer extra comfort and put-downs. Just try to keep the same sequence: same bath, same feed, same story, same song, same final put-down sequence. The routine is a container that holds sleep — keep it intact even during rough patches.
What Doesn't Help
Amber teething necklaces. There is no scientific evidence that amber teething necklaces reduce teething pain. Beyond being ineffective, they're a strangulation and choking hazard. Both the AAP and multiple pediatric safety organizations recommend against them.
Increasing feeds at night "just in case." If your baby was previously sleeping in longer stretches and is now waking, it can feel intuitive to offer extra night feeds in case hunger is involved. But offering feeds at every waking during a teething stretch can create new feeding associations that persist long after the teeth have come in. Assess whether your baby is actually hungry (rooting, strong feeding cues) or uncomfortable and looking for comfort, and respond accordingly.
Cutting the routine short. Putting a baby to bed "easier" during teething by skipping wind-down steps often backfires. The wind-down routine reduces cortisol and signals the body to prepare for sleep — a baby who goes into the crib without that preparation is harder to settle, not easier.
Assuming every rough night is teething. If your baby has been in a rough stretch for more than 2–3 weeks with no visible tooth progress, it's worth reconsidering whether something else is happening — a sleep regression, a new developmental leap, a feeding change, or emerging sleep associations.
How Long Does It Last?
Individual teething episodes — the window when a specific tooth is actively cutting through — typically cause peak discomfort for 3–7 days. Once the tooth breaks through the gum surface, the pressure and inflammation drop quickly and sleep usually bounces back within a day or two.
What makes teething feel longer than it is: teeth rarely come in one at a time. Multiple teeth often erupt in sequence or simultaneously, so you can have a run of 2–4 weeks where there's always some tooth in active eruption. During these stretches, a cycle of cold teethers + appropriate pain relief on the roughest nights + maintaining your routine is the most effective strategy.
The full teething timeline stretches to about 2.5–3 years, but the vast majority of that time is quiet. The noisy periods — when eruption is actively happening — are genuinely short. This will not last forever.
Red Flags: When to Call the Pediatrician
Most teething is managed at home. But certain symptoms mean you should reach out:

Drool rash — the red, irritated skin on your baby's chin, cheeks, or chest from constant moisture — is normal and not a red flag. Keep the area dry with gentle patting, and apply a thin barrier cream (petroleum jelly or zinc-based) to protect the skin.
The Bottom Line
Teething is real, and it genuinely disrupts sleep for a few days at a time as each tooth emerges. But it's also one of the most over-blamed causes of sleep disruption in the first year — which means many parents endure weeks of rough sleep waiting for teething to pass when something else (a regression, a new habit, a developmental leap) is actually driving the problem.
